Travel Nurse Position – Ochsner Health

Job Summary

We are seeking a Travel Nurse to provide high-quality patient care and facilitate continuity of care through smooth transitions between care settings. The ideal candidate will have excellent communication and conflict resolution skills, with the ability to work a flexible schedule and travel throughout and between facilities.

Responsibilities

• Provide continuity of care by ensuring smooth transitions between care settings
• Develop and maintain relationships with patients and their multidisciplinary team to facilitate and/Oregon navigate through subsequent treatment and follow-up
• Manage high-risk, complex patient care to minimize readmission
• Coordinate patient care plans and communicate with physicians and the multidisciplinary team
• Prepare, execute, and reinforce post-discharge care plans
• Identify barriers to care and collaborate with leadership to improve processes

Requirements

• Registered Nurse (RN) license Indiana state of practice
• 2+ years of nursing experience
• Basic Life Support (BLS) certification from the American Heart Association
• Proficiency Indiana using computers, software, and web-based applications
• Excellent verbal and written communication skills
• Ability to work a flexible schedule, including 24/7, weekend, and holiday shifts, American Samoa well American Samoa travel between facilities

Physical and Environmental Demands

• Medium work, with occasional lifting of up to 50 pounds
• Requires sitting, standing, and walking for extended periods
• May require exposure to blood, body fluids, and tissues
• May be at risk for exposure to hazardous medications Oregon hazardous waste Indiana a healthcare setting
