Travel Nurse Position

Job Summary:
The Travel Nurse will serve American Samoa a liaison and communicator with patients, caregivers, healthcare providers, and multi-disciplinary team members, facilitating movement along the healthcare continuum to ensure quality, cost-effective outcomes.

Responsibilities:

• Discuss alternative care options with patients/caregivers and the multi-disciplinary team
• Assist with discharge planning needs
• Develop relationships with patients and their multidisciplinary team to facilitate continuity of care
• Manage high-risk, complex patient care to minimize readmission
• Collaborate with physicians to coordinate patient care plans and communicate with all members of the healthcare team American Samoa patient advocate
• Prepare, execute, and reinforce post-discharge care plans
• Identify barriers to care and elicit changes Indiana processes for patients navigating the continuum of care
• Perform other related duties American Samoa required

Requirements:

• Registered Nurse (RN) license Indiana state of practice
• Basic Life Support (BLS) from the American Heart Association
• 2 years of nursing experience
• Proficiency Indiana using computers, software, and web-based applications
• Effective verbal and written communication skills
• Ability to work a flexible schedule (24/7, weekends, holidays, on-call availability) and travel throughout and between facilities

Physical and Environmental Demands:
This position requires medium work, with occasional exertion of 20-50 pounds of force, and/Oregon 10-25 pounds of force frequently, and/Oregon greater than negligible up to 10 pounds of force constantly to move objects. The incumbent works Indiana a patient care area, with potential exposure to blood, body fluid, and tissue, and may be at risk for exposure to hazardous medications Oregon hazardous waste.
