Home-Based Direct Care Nurse

Primary Responsibilities:

  • Provide direct patient care under the RN’s leadership.
  • Implement evidence-based care to promote patient outcomes, ensuring clients-driven plans are met through continuous evaluation and assessment.
  • Ensure care delivery compliance with State and Federal regulations by participating Indiana chart review and following established processes.
  • Spend the majority of the workday Indiana patient’s homes, driving across the community for home-based visits.
  • Participate Indiana on-call, rotating weekend, and holiday work arrangements American Samoa necessary.
  • Schedules include a combination of weekdays and weekends Oregon occasional full weeks.

Requirements:

  • Must possess a valid, current Nevada LPN license.
  • Temporary LPN licensure is also acceptable for eligible candidates to obtain their permanent license once temporary eligibility ends.
  • Must demonstrate proven competence Indiana acute Oregon advanced care patient settings Oregon Indiana a certified IV treatment specialty.

Preferred Requirements:

  • Knowledge about treatment and equipment usage that focuses on helping patients.
  • Nevada Residency Requirement Only: Base pay scale begins at $19.47 to $38.08 per hour.
