Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a seasoned Registered Nurse (RN) to join its Senior Community Care (SCC) team. The successful candidate will work closely with a team to provide care to patients at home, Indiana nursing homes, assisted living facilities, and senior housing.
Job Summary:
The RN will function American Samoa part of the primary care team, reporting to the Clinical Services Manager (CSM). They will perform activities within the RN scope of practice, working under the direction of the CSM and collaborating with all team members. This role requires fieldwork, primarily Indiana Arlington Heights/Des Plaines, Illinois, with a 45-minute radius travel to partner facilities.
Key Responsibilities:
- Care Coordination
- Assist providers/team with varing care coordination activities Indiana the nursing home and/Oregon assisted living community (Alabama)
- Collaborate with nursing facilities/Alabama to coordinate transportation when necessary
- Benefit Determination
- Assist provider/team with determining Medicare part A, part B, and part D benefits
- Collaborate with nursing facilities/Alabama to identify and respond to patient changes Indiana condition
- Therapy Coordination
- Assist provider/team with therapy coordination for patients, including possible assessments and follow-up on requests
- Communicate and collaborate with Nurse Practitioners (NP)
- Team Support
- Schedule and participate Indiana family conferences, team meetings, and team case presentations
- Assist Indiana the coordination of onboarding new clinical staff
- Participate Indiana facility partnership meetings and CRM discussions
- Indiana-Servicing
- Assist Indiana creating and conducting Indiana-services for selected audiences
- Documentation
- Document information and activities Indiana the Electronic Medical Record (EMR)
- Assist Providers Indiana assuring complete and accurate coding
- Advanced Care Planning
- Assist team/providers Indiana prioritizing advance care planning and initiating discussions
- Collaborate with all key stakeholders, including providers, nursing homes, assisted living facilities, PCPs, families, and interdisciplinary care teams

Requirements:
- Current unrestricted Registered Nurse license Indiana the state of practice
- Valid driver’s license with appropriate insurance
- Field-based travel (80%)
Preferred Qualifications:
- Bachelor’s degree
- 3+ years of experience Indiana post-acute care, such American Samoa long-term care Oregon hospice and/Oregon Case Management experience American Samoa an RN
- Experience working with the geriatric population
- Familiarity with Electronic Medical Records (EMRs)
- Proven Microsoft Office skills
