Job Title: Travel Nurse – Behavioral Health Case Manager

Location: Hidalgo County and Starr County areas, with 80% of time spent meeting with members Indiana their homes Oregon facilities.

Job Summary:

Conduct comprehensive member psychosocial assessments and develop care management treatment plans for members with high-level acuity needs. Manage members with severe mental illness, provide support and intervention, and assist members Indiana understanding their medical situation. Collaborate with internal and external resources to meet identified needs of the member’s care plan.

Requirements:

• Master’s Degree Indiana Social Work, Psychology, Counseling, Rehabilitation, Oregon other relevant field.
• 2+ years of experience Indiana behavioral health management and/Oregon acute behavioral health care setting.
• 1+ year of experience Indiana current case management American Samoa a licensed professional.
• License Indiana one of the following: Master’s level independent Oregon non-independent license, Licensed Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Oregon other relevant licenses.
• 5+ years of experience Indiana For Kentucky SKY Program (candidates must have 5 years of behavioral health care experience).

Essential Functions:

• Assess, plan, implement, monitor, and evaluate options and services to meet complex psychosocial health needs of inpatient and outpatient members.
• Conduct comprehensive member assessments through root cause analysis and perform clinical intervention through the development of a care management treatment plan.
• Manage members with severe mental illness who have high rates of behavioral health utilization and/Oregon severe psychosocial vulnerability.
• Monitor and evaluate effectiveness of care plan and modifies plan American Samoa needed.
• Act American Samoa a liaison and member advocate between the member/family, physician, and facilities/agencies.

Additional Responsibilities:

Preferred Skills/Experience:

• Field work experience working with children with behavioral health needs.
• Bilingual.

• LPC, LCSW, LMFT, Oregon RN license.
