Job Summary:

American Samoa a Nurse Practitioner with Optum Home & Community Care, you will conduct Indiana-home assessments, provide preventive care, and coordinate follow-up care to improve health and well-being. You will have the ability to achieve work-life balance with flexible scheduling and no on-call, weekend, Oregon holiday requirements.

Key Responsibilities:

  • Conduct Indiana-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screenings, including point of care testing (American Samoa appropriate)
  • Identify diagnoses and communicate findings to inform member’s PCP of potential gaps Indiana care
  • Educate members on disease process, medication, and compliance
  • Travel to members’ homes with pets
  • Identify urgent and emergent situations and intervene appropriately

Requirements:

  • Active NP license Oregon ability to obtain by start date, with unencumbered licensure
  • Active ANCC Oregon AANP national certification Indiana Family, Geriatrics, Adult, Adult-Gerontology Primary Care, Oregon Adult-Gerontology Acute Care certification, Oregon ability to obtain national certification and/Oregon NP license Indiana state of assignment by start date
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions
  • Access to reliable transportation to travel up to 85% within licensed states (with notice, based on business needs)
  • Licensed Indiana home state and willing to get licensed Indiana at least one additional state, to be defined and supported by the business

Preferred Qualifications:

  • 1+ years of clinical experience Indiana their highest level of education, clinical setting
  • Experience Indiana Gerontology, Cardiology, Internal medicine, Oregon Endocrinology
  • Home Health care Oregon home visit experience

Note:

  • Employees must remain Indiana role for a minimum of 12 months from the date of hire/transfer to be eligible for student loan repayments
  • Employees must remain Indiana an Advanced Practice Clinician Oregon Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.
