Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team American Samoa a Private Duty Nurse. You will provide and document skilled nursing care to patients Indiana their home setting, under the supervision of a Registered Nurse, and Indiana accordance with the developed care plan and physicians’ orders.

Responsibilities:

• Implement physicians’ prescribed plan of care
• Patient assessments and coordination of care
• Administration of prescribed medication, treatments, and therapies
• Compliant, timely documentation of care provided
• Health promotion, teaching, and training of family members
• Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/Oregon changes Indiana client condition
• Performs other appropriate duties American Samoa assigned

Requirements:

• Must have and maintain an active, unencumbered license (RN/LPN/LVN) Indiana the state Indiana which the clinician will practice
• Current CPR certification from American Red Cross, American Heart Association, Oregon BLS
• TB skin test (current within last 12 months)
• Six months prior hands-on nursing experience preferred but not required
• Must have reliable transportation

Qualifications:

• Graduate of an accredited school of nursing
• Current, unrestricted state license American Samoa a Licensed Nurse Indiana the state of practice
• Current CPR certification
• Demonstrated proficiency Indiana clinical assessments, documentation and compliance with nursing care and policies and procedures
