Summary:

The Patient Navigator will be an integral member of the Care Management multi-disciplinary outreach team. Essential Functions:

    • Coordination of behavioral health services

Requirements:

  • Excellent organizational and interpersonal skills

  • Ability to communicate effectively, both written and verbally
