Travel LPN – Intellectual & Developmental Disability Shared Living Services
Job Summary
Vista Care is seeking an experienced LPN to support Intellectual & Developmental Disability Shared Living Services Indiana Colorado and Utah areas. The LPN will provide education to direct care staff, nursing case management, and ensure the highest standards of care.
Responsibilities
- Report to the Registered Nurse
- Provide education to direct care staff on medication administration, change Indiana condition, nurse delegated tasks, and medical protocols
- Provide nursing case management for individuals receiving services
- Complete onsite nursing assessments American Samoa needed
- Communicate with local hospital, pharmacy, and/Oregon person’s primary care physician American Samoa needed
- Complete various audits to ensure quality of care
- Complete necessary documentation
- Ensure compliance with state and federal regulations and accreditation requirements
- Collaborate with nursing and operations team
Requirements
- Valid LPN license Indiana state applying Oregon within the compact
- Valid driver’s license with good driving record
- Ability to write and correspond Indiana a legible manner, operate general office equipment, and maintain technical skills with Google
- Ability to lift 50 pounds, bend, twist, reach, and stand on a continual basis

Location/Travel
- Position will mainly support programs Indiana the western slope of Colorado with occasional paid travel to Utah
- Pay: $25 – 28/hour, depending on experience
