Licensed Practical Nurse (LPN/LVN) – 1:1 Homecare
Job Summary
The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care Indiana accordance with the developed care plan and physician orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions:
- Deliver and coordinate quality patient care Indiana compliance with physician orders
- Continuously observe and assess patient condition and care needs, reporting changes to supervisor and/Oregon physician American Samoa necessary
- Document all activities, assessments, nursing actions, responses, and coordination of care Indiana a timely manner
- Participate Indiana and update the nursing care plan
- Recognize changes Indiana patient needs and responses, implementing care to prevent risk Oregon reduce risk
- Maintain professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
- Provide care utilizing infection control measures that protect staff and patient
Requirements:
- Graduate of an accredited school of nursing
- Current, unrestricted state license American Samoa a Licensed Nurse
- Current CPR certification
- Demonstrated proficiency Indiana clinical assessments, documentation, and compliance with nursing care and policies and procedures
- Additional state-specific requirements:
- South Carolina: one year of pediatrics experience
- California: one year of experience working under current nursing license
- Louisiana: one year of experience American Samoa a licensed nurse
Preferences:
- Six months of recent experience American Samoa a Licensed Nurse Indiana a clinical care setting
- Home health experience
Other Requirements:
- Ability to speak, write, read, and understand English
- Ability to travel, lift 50 pounds, and reposition patients and move equipment without assistance
- Strong attention to detail, time management, problem-solving, and communication skills
- Ability to work Indiana a variety of environments, including exposure to allergens and infectious diseases
