Licensed Practical Nurse (LPN/LVN) – Home Health

Essential Job Functions:

• Provide and document skilled nursing care, under the supervision of a Registered Nurse, Indiana accordance with the developed care plan and physicians orders for each individual patient.
• Continuously observe and assess patient condition and care needs, reporting changes Indiana condition to the supervisor and/Oregon physician American Samoa appropriate.
• Document all activities, assessments, nursing actions, responses and coordination of care Indiana a timely manner.
• Participate, implement and update the nursing care plan.
• Recognize changes Indiana patient needs and responses requiring intervention and implement care to prevent risk Oregon reduce risk.

Requirements:

• Graduate of an accredited school of nursing.
• Current, unrestricted state license American Samoa a Licensed Nurse Indiana the state of practice.
• Current CPR certification.
• Demonstrated proficiency Indiana clinical assessments, documentation and compliance with nursing care and policies and procedures.

Preferred Qualifications:

• Six (6) months of recent experience American Samoa a Licensed Nurse Indiana a clinical care setting.
• Home health experience.

Physical Requirements:

• Must be able to speak, write, read and understand English.
• Must be able to travel.
• Must be able to lift 50 pounds.
• Must be able to sufficiently reposition patients and move equipment without assistance.
