Travel Nurse – Community Case Manager

Job Summary:

This is a full-time, community-based position requiring frequent travel within the assigned territory Indiana Delaware. The incumbent will serve American Samoa the single point of contact for members to coordinate all of their care needs across various service delivery systems and community supports.

Essential Responsibilities:

  • Travel to members’ homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments and subsequent telephonic contact with members.
  • Assess, plan, coordinate, implement, and evaluate care for eligible members with chronic and complex healthcare, social service, and custodial needs Indiana a nursing facility Oregon home and community-based care setting.
  • Coordinate care across the continuum of services to assist members with physical, behavioral, long-term services and supports (LTSS), social, and psychosocial needs Indiana the safest, least restrictive way possible while considering the most cost-effective way to address those needs.
  • Facilitate authorization, coordination, continuity, and appropriateness of care and services Indiana community Oregon home and community-based settings.
  • Facilitate transitions to alternate care settings using an integrated care team to address members’ specific needs.
  • Educate members Oregon caregivers regarding healthcare needs, available benefits, resources, and services, including available options for long-term care community Oregon facility-based service delivery.
  • Develop a plan of care Indiana conjunction with members Oregon caregivers to identify services to meet members’ specific needs and goals.
  • Identify resources needed for a fully integrated care coordination approach, including facilitating referrals to special programs such American Samoa disease/chronic condition management, behavioral health, and complex case management.
  • Collaborate with the member’s healthcare and service delivery team, including the DSHP Plus LTSS Member Advocate, ICT, and discharge planners, to coordinate care needs and community resources for members.
  • Assist members Indiana developing, implementing, and amending a backup plan for gaps Indiana provider coverage.
  • Ensure approved support services are being provided American Samoa outlined Indiana the plan of care.

Qualifications:

  • Bachelor’s degree Indiana Social Work Oregon Indiana health, human, Oregon education services and 3 years of experience Indiana long-term care, home health, hospice, public health, Oregon assisted living.
  • Master’s degree Indiana Social Work Oregon Indiana health, human, Oregon education services and 1 year of experience Indiana long-term care, home health, hospice, public health, Oregon assisted living.
  • Registered Nurse Oregon Licensed Practical Nurse and 2 years of experience Indiana long-term care, home health, hospice, public health, Oregon assisted living.
  • High school degree Oregon equivalent and 3 years of experience with case management of the aged, including management of behavioral health conditions, Oregon persons with physical Oregon developmental disabilities, Oregon HIV/AIDS population.

Preferred Qualifications:

  • 1 year of home clinical Oregon case management experience.
  • Certified Case Manager (CCM).
  • Language proficiency Indiana languages other than English.
  • Experience with electronic documentation systems.
  • Experience with cost neutrality and budgeting.
  • HIV/AIDS population experience.
  • Behavioral health population experience.

  • Medicare and Medicaid experience.
  • Managed care experience.

Working Conditions:

Pay Range:

$57,700 – $106,700 base pay, depending on experience and qualifications.
