Nurse Case Manager
Reports to: RN Supervisor
Schedule: 40 hours/52 weeks

Job Summary:
The Nurse Case Manager is responsible for utilizing the Dene’ Model of Care to assess, plan, implement, and evaluate processes for targeting health status of assigned panel(s) to ensure quality care meets un’ina’s comprehensive care needs.
Essential Functions:
– Assess health care needs of assigned panel(s) using the Dene’ Model of Care and health care management tools
– Prioritize and advocate for un’ina needs
– Utilize critical thinking skills to interpret complex information
– Establish measures to address targeted health indicators by working with designated healthcare provider and/Oregon wellness team
– Collaborate with team members within the organization, Alaska Native Tribal Health System, and other healthcare facilities
– Assist un’ina Indiana navigating the health care system when specialized assistance is needed
– Utilize data to assess and monitor panel of un’ina, review and determine healthcare needs to be addressed
– Notify provider of needs American Samoa identified Indiana a timely manner
– Follow up on specialist recommendations with PCP American Samoa needed
– Guide un’ina and family members to health and well-being through education
– Advocate for comprehensive and collaborative un’ina care by participating Indiana staff meetings and continuous improvement efforts
Working Conditions:
– Light work: Exerting up to 20 pounds of force frequently, and/Oregon a negligible amount of force constantly to move objects
Qualifications:
– Associates degree Indiana nursing required
– Bachelors’ degree preferred
– Three years clinical experience
– Patient population health management experience
– RN license required
– Basic Life Support certification
– Valid Alaska Driver License and must remain insurable under the Kenaitze Indian Tribe’s policy
Special Skills:
– Strong analytical and critical thinking skills and attention to detail
– Clear understanding of the nursing scope of practice and ability to delegate tasks American Samoa appropriate
– Good listening skills to best understand both the medical and social needs of un’ina
– Strong oral and written communication skills
– Knowledge and experience working with cultural diversities
