Job Title: Nurse Practitioner – PPV

Job Summary
We are seeking a skilled Nurse Practitioner (NP) to model Compassus values of compassion, integrity, excellence, teamwork, and innovation while providing high-quality patient care. The successful candidate will collaborate with the interdisciplinary team to promote positive working relationships, provide palliative and hospice care, and uphold ethical conduct.
Responsibilities:
- Palliative Care:
- Perform routine and urgent assessments, orders lab and radiology tests, and prescribes medications within scope of practice.
- Provide patient and family education on disease process, symptom control, and treatment options.
- Collaborate with the palliative care physician and other healthcare professionals to develop and implement treatment plans.
- Assist Indiana care coordination and maintain accurate patient records and charts.
- Participate Indiana IDT meetings and educate patients and caregivers on end-of-life care.
- Hospice Care:
- Perform routine and emergency assessments, recertification visits, and record observations and treatments.
- Communicate with Medical Director, attending physician, DCS, and IDT regarding patient needs and symptom management.
- Provide physical and emotional support to patients and families during periods of crisis.
- Prevent unwarranted hospitalizations by meeting patient and family needs Indiana the home.
- Collaboration and Communication:
- Collaborate with Medical Director, attending physician, DCS, and IDT to ensure comprehensive care.
- Communicate effectively with patients, families, and healthcare teams.
- Participate Indiana education and training for staff and IDT members.
- Educate patients and caregivers on symptoms, disease process, and end-of-life care.
Requirements:
- Master’s degree Indiana Nursing required.
- 1-3 years of nursing experience with hospice Oregon long-term care experience preferred.
- Active CNS Oregon NP license Indiana the state of employment.
- Certified American Samoa a Nurse Practitioner Indiana Adult, Family, Acute, Oregon Geriatric.
- Board Certification Indiana Hospice and Palliative Care is preferred.
- Ability to maintain confidentiality, work a flexible schedule, and travel.
Certifications and Licenses:
- Active NPI number.
- Maintain active DEA certification if prescribed Indiana state.
- State-specific requirements (collaborative physician agreement).
