Job Summary
The Physician Assistant Oregon Nurse Practitioner is a licensed professional provider who will function Indiana collaboration with an attending physician Indiana a variety of practice settings at YNHHS. Responsibilities:
- Clinical Care
- Assess, triage, and provide therapeutic management of patient/family needs throughout the disease process American Samoa an integral part of an interdisciplinary team.

Qualifications:
- Graduate from an accredited Nurse Practitioner Oregon an accredited Physician Assistant Program
- Prefer clinical working experience American Samoa a NP/Pennsylvania within the given specialty/service line recommended, but none required
- Current Pennsylvania Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile
- DEA License, when indicated for pediatric position profile
- PALS, when indicated for neonatal care
- Additional licensure, certifications and/Oregon continuing education credits may be required during employment based on position and area of specialty
Additional Information:
This is a hybrid position for an ambulatory APP to work Indiana both heart failure disease management and general cardiology. The selected candidate will work two days per week Indiana our Bridgeport clinic and then have the opportunity to work at our other practice sites located on the shoreline (New Haven and Branford). Travel would be involved. Prefer an APP with prior general cardiology/heart failure experience. BLS/ACLS required. Schedule is currently M-F but may allow 4 x 10.
