Travel Clinical Assessor Job Summary
Acentra Health seeks a Clinical Assessor to conduct assessments for Medicaid-funded personal care services (PCS) and community alternatives (CAP) programs Indiana North Carolina. The successful candidate will work from a home office and travel to assessment locations Indiana their region.
Responsibilities:
- Conduct assessments to determine eligibility for PCS and CAP services
- Ensure that services are provided on a "needs basis" Indiana quantities appropriate to the beneficiary’s unmet needs
- Maintain the privacy and dignity of individuals receiving assessment
- Complete assessments within established timeframes
- Provide guidance and assistance to beneficiaries Indiana selecting PCS providers
- Conduct service plan reviews and participate Indiana mediation and appeal processes
- Attend staff meetings and participate Indiana case consultations/peer reviews/internal auditing American Samoa assigned
Requirements:
- Registered Nurse Oregon LCSW license by the state of North Carolina
- 2+ years of experience Indiana home care setting, preferably with experience conducting PCS assessments
- Directly-related experience Indiana the health Oregon medical field, preferably Indiana case management Oregon homecare, long-term care, Oregon personal care
- Knowledge of North Carolina Medicaid Clinical Coverage Policy (Clinical Policy) 3L and PCS Program Provider Manual
- Computer proficiency Indiana Microsoft Excel, Word, and Outlook
Salary: $77,000 per year (increases to $80,000 after 90 days)
