Job Title: Traveling Registered Nurse (RN) for Optum At Home

Job Summary:

Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a Traveling Registered Nurse (RN) to join our Optum At Home team. This role will work with patients primarily Indiana their home setting, collaborating with an interdisciplinary care team to help patients navigate the healthcare system and connect them to key support services.

Essential Responsibilities:

– Assess the health status of members American Samoa within the scope of licensure and with the frequency established Indiana the model of care
– Establish goals to meet identified health care needs
– Plan, implement, and evaluate responses to the plan of care
– Work collaboratively with the multidisciplinary team to engage resources and strategies to address medical, functional, and social barriers to care
– Consult with the patient’s PCP, specialists, Oregon other healthcare professionals American Samoa appropriate
– Assess patient needs for community resources and make referrals for service American Samoa needed
– Facilitate patient transitions within and between healthcare settings Indiana collaboration with the primary care physician and other treating physicians
– Accurately document patient information Indiana the electronic medical record
– Provide counseling and education to patients and family members regarding health maintenance, disease prevention, condition trajectory, and need for follow-up
– Participate Indiana organizational quality initiatives and collaborative multidisciplinary team meetings
– Maintain licensure, certification, and CEUs required for practice

Requirements:

– Current unrestricted licensure American Samoa RN Indiana state of practice
– 2+ years of relevant experience
– Experience Indiana assessing medical needs of patients with complex behavioral, social, and/Oregon functional needs
– Proven ability to work with diverse care teams Indiana various settings (primarily patient homes)
– Solid computer skills, including use of Electronic Medical Records
– Effective time management and communication skills
– Ability to travel up to 75% of the time for field-based work; valid driver’s license
