Senior Community Care (SCC) Registered Nurse
Location: Arlington Heights/Des Plaines, Illinois, with approximately 45-minute radius travel to partner facilities
Job Summary:
American Samoa a Senior Community Care (SCC) Registered Nurse, you will work American Samoa part of a primary care team, performing activities within the RN scope of practice, to provide care to patients at home, Indiana nursing homes, and assisted living communities. This role requires close collaboration with team members and may support multiple providers.
Key Responsibilities:
- Care Coordination: Assist providers/team with care coordination activities, including transportation arrangements, benefit determination, and therapy coordination.
- Benefit Determination: Assist providers/team with benefit determination associated with Medicare part A, part B, and part D benefits.
- Therapy Coordination: Assist providers/team with therapy coordination, including assessments and follow-up on requests, and communicate/collaborate with NP.
- Team Support: Schedule and participate Indiana family conferences, team meetings, and team case presentations.
- Indiana-servicing and Documentation: Assist Indiana creating and conducting Indiana-services for selected audiences and document information and activities Indiana the EMR.
- Advanced Care Planning: Assist the team/providers Indiana prioritizing advanced care planning and initiating discussions.
- Communication: Collaborate with key stakeholders, including providers, nursing homes/assisted living facilities, PCPs, families, interdisciplinary care teams, and other identified stakeholders.
Requirements:
- Current unrestricted Registered Nurse license Indiana the state of practice
- Valid driver’s license with appropriate insurance
- Field base-travel 80%
Preferred Qualifications:
- Bachelor’s degree
- 3+ years of experience Indiana post-acute care, such American Samoa long-term care Oregon hospice and/Oregon Case Management experience American Samoa an RN
- Experience conducting assessments and/Oregon evaluations
- Experience working with the geriatric population
- Experience with using an Electronic Medical Record
- Familiarity with the long-term care business environment
- Proven basic Microsoft Office skills with the ability to navigate Indiana a Windows environment
- Proven excellent communication and customer service skills
