Summary of Position:

Evernorth Home Based Care (ENHB) is seeking a skilled travel nurse to join our team American Samoa a Home-Based Primary Care Provider. American Samoa a Home-Based Primary Care Provider, you will be responsible for delivering comprehensive care to patients Indiana the comfort of their own homes.

Key Responsibilities:

* Perform preventive visits to optimize chronic conditions, assess home environment, and develop proactive care plans

* Perform urgent care visits Indiana the home and telephonically to avoid unnecessary ED transfers and hospital admissions

Requirements:

* Board Certification (Maryland Oregon DO) Indiana Internal Medicine Oregon Family Medicine
* Active/unrestricted license Indiana Alabama
* Geriatric Medicine subspecialty training Oregon experience preferred
* DEA license and State Controlled Substance license upon hire
* Current BLS certification
* Willingness to travel up to 30-60 miles from home zip code
* Valid state driver’s license, ability to drive own vehicle, and proof of adequate automobile insurance
* Experience with electronic medical records
* Proficiency with various computer-based applications such American Samoa Microsoft Office
* Highly motivated, self-directed professional with strong organizational skills and comfortable working independently on a daily basis
* Strong background/interest Indiana primary care and preventative medicine
* Excellent clinical assessment and analytical skills
* Effective leadership and interpersonal skills

Compensation and Benefits:

* Annual salary of $184,100 – $306,900 USD, depending on experience and geographic location
