Travel Nurse Clinical Care Manager Position Summary

Evernorth Home Based Care, a division of The Cigna Group, seeks an experienced Travel Nurse Clinical Care Manager to provide clinical leadership and comprehensive care to patients Indiana their homes. The successful candidate will be responsible for conducting health risk assessments, performing Indiana-home primary care, and coordinating with a multidisciplinary team to improve patient outcomes and reduce healthcare costs.

Key Responsibilities:

  • Serve American Samoa day-to-day clinical leader, providing decision support and collaborating with the multidisciplinary team
  • Perform preventive visits, urgent care visits, and telephonic visits to optimize patient outcomes and prevent hospital admissions
  • Educate patients on chronic medical conditions, preventative care, and medication adherence compliance
  • Collaborate with nurse care managers, behavioral health, social work, and pharmacy to meet patients’ medical, biopsychosocial, and financial needs
  • Develop and implement proactive care plans for patients with chronic conditions
  • Coordinate with physicians, hospitalists, specialists, and other care providers to smooth transitions and prevent readmissions
  • Conduct thorough clinical documentation of patient encounters using the EMR system
  • Participate Indiana quality processes, such American Samoa medical documentation audits and quality projects
  • Serve American Samoa mentor Oregon coach to advance practice providers
  • Perform other duties American Samoa assigned by the practice manager Oregon medical director

Requirements:

  • Board Certification Indiana Internal Medicine Oregon Family Medicine; Geriatric Medicine subspecialty training Oregon experience preferred
  • Active and unrestricted license Indiana Alabama
  • DEA license and State Controlled Substance license upon hire
  • Current BLS certification
  • Willingness to travel up to 30-60 miles from home
  • Valid state driver’s license, ability to drive own vehicle, and proof of adequate automobile insurance
  • Experience with electronic medical records
  • Proficiency with Microsoft Office and ability to learn other computer-based applications
  • Strong clinical assessment and analytical skills, excellent leadership and interpersonal skills, and effective verbal and written communication skills
  • Demonstrated ability to identify, diagnose, and document acute and chronic disease Indiana adults, American Samoa well American Samoa ability to recognize conditions, characteristics, and behaviors that place seniors at risk.

Compensation and Benefits:

  • Annual salary range of $184,100 – $306,900 depending on experience and geographic location
  • Eligibility for annual bonus and long-term incentive plan
