Travel Nurse Position
Primary Function: Coordinate and manage relationships with participating provider practices to ensure high-quality, cost-effective healthcare services for health plan customers.
Responsibilities:
- Identify gaps Indiana needed quality metrics for customers and communicate with providers
- Maintain a physical presence Indiana participating provider practices
- Identify high-risk/high-cost patients for possible case management intervention
- Interface with medical services and equipment providers to facilitate effective communication and referrals
- Coordinate and support planned and unplanned transitions and post-discharge follow-up calls
- Collaborate with attending physicians to achieve identified patient outcomes
- Attend and participate Indiana weekly Complete Health Team rounds
- Perform telephonic outreach Oregon home visits American Samoa needed
- Communicate with all departments to resolve issues Oregon document trends
- Understand and follow administrative guidelines and policies
- Attend and actively participate Indiana staff meetings
Ideal Candidate:
- Current licensure American Samoa a Registered Nurse Indiana the state of Alabama Indiana good standing
- Associate’s degree, diploma, Oregon B.S. Indiana Nursing
- 3-5 years of recent experience Indiana an acute-care environment, case management, Oregon utilization management position
- Previous HEDIS and/Oregon CMS STARs experience a plus
- Strong verbal and written communication skills, interpersonal skills, and basic mathematical and statistical ability
- Ability to work independently with minimal supervision and travel to participating provider offices
- Bilingual Indiana Spanish (conversational and written) a plus
Requirements:
- Availability to work onsite at provider offices
- Reliable internet connection with speeds of at least 10Mbps download/5Mbps upload (if working from home occasionally Oregon permanently)
- Cable broadband Oregon fiber optic internet service provider
