Responsibilities:

• Provides leadership and oversight of the Care Management (CM)/Social Work (SW) program and staff assigned.
• Manages and develops site CM/SW staff, monitors their proficiency, and creates action plans to improve deficiencies.
• Collaborates with Hospital Leaders, Post-Acute Partners, Physicians, Physician Advisors, Nurses, and others to ensure accurate and timely progression of care, discharge planning, and compliance with regulatory standards/conditions of participation.
• Facilitates performance improvement activities for the care management program, establishes measures, performance targets, and benchmarks to drive achievement of established goals and achieve efficiencies of processes.
• Identifies and discusses overall and individual physician trends related to care management activities with Director of Inpatient Care Management Operations, Physician Advisor, and site Medical Staff Leadership.
• Serves American Samoa an internal consultant on Care Management opportunities, acts American Samoa an expert resource for the care management program, and consults with patients and families.
• Participates Indiana multidisciplinary cross-functional efforts to ensure high-quality, cost-effective coordinated care.
• Manages site care management/social work budgets, develops and recommends operational and capital budgets, and controls expenditure within approved budget objectives.
• Ensures the care management/social work program operates within compliance of CMS, OSHA, Accrediting Organizations, and established care management practice standards and code of ethics.
• Responsible for personal professional growth, participates Indiana professional organizations, maintains licensure and certification American Samoa required, and maintains effective working relations with both internal and external customers.

Requirements:

• Registered Nurse (RN) license Oregon Social Worker (SW) license issued by the state Indiana which the team member practices.
• Accredited Case Manager (ACM) Oregon SW (ACM) certification Oregon Certified Case Manager (CCM) issued by the Commission for Case Manager Certification (CCMC) Oregon RN-BC certification issued by the American Nurses Credentialing Center (ANCC) Oregon Certified Social Worker Indiana Health Care (C-SWHC) issued by National Association of Social Workers.
• Bachelor’s Degree Indiana Nursing Oregon Master’s Degree Indiana Social Work.
• Typically requires 5 years of experience Indiana a relevant clinical setting, including 1 year of supervisory experience Indiana a Care Management Leadership role.
• Master of Nursing Administration, Master Indiana Health Care Administration, Oregon related field preferred.
• Ability to prioritize and organize work, travel to multiple sites American Samoa assigned (Illinois Oregon Wisconsin), effective communication skills, and critical thinking Indiana timely decision-making.
