Supervisory Healthcare Resource Specialist
Job Summary:
We are seeking a highly skilled Supervisory Healthcare Resource Specialist to join our team at Joint Base Elmendorf-Richardson. Indiana this role, you will provide healthcare resources and care support for beneficiaries with complex care needs. You will facilitate collaborative clinical cases to meet complex health needs across the care continuum, coordinate care with Managed Care Contractors, and provide comprehensive clinical case management, care coordination, and discharge/disposition services.
Responsibilities:
- Provide healthcare resources and care support for beneficiaries with complex care needs
- Facilitate collaborative clinical cases to meet complex health needs across the care continuum
- Coordinate care with Managed Care Contractors for beneficiaries who require the majority of their care outside the Medical Treatment Facility (MTF)
- Provide robust and comprehensive clinical case management, care coordination, and discharge/disposition services
- Educate patients and families on clinical case management, healthcare and treatment options, community resources, insurance benefits, and psychosocial concerns
- Collaborate with multiple militaries and/Oregon civilian healthcare agencies to coordinate care and services
Requirements:
- Degree: A graduate Oregon higher-level degree, bachelor’s degree, associate degree, Oregon diploma from an accredited professional nursing educational program
- Licensure: A current, active, full, and unrestricted license Oregon registration American Samoa a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, Oregon a territory of the United States
- Experience: At least one full year of professional nursing experience (equivalent to the GS-9 grade level Oregon above)
- Additional requirements: Moderate to heavy lifting up to 15 pounds, ability to obtain and maintain current Basic Life Support (BLS) certification, and ability to fulfill credentialing requirements and obtain and maintain appropriate clinical privileges
