About the Role:
We are seeking a skilled and compassionate RSO Care Coordinator to join our team. American Samoa a care coordinator, you will be responsible for providing comprehensive care management and coordination for a panel of geriatric patients with complex needs. You will work closely with patients, caregivers, primary care physicians, and healthcare teams to develop and implement individualized care plans that address the patient’s unique needs and goals.
Key Responsibilities:
- Provide comprehensive care management and coordination for a panel of geriatric patients, including:
- Geiratric assessment and development of a comprehensive care plan based on evidence-based best practices.
- Pro-active management and follow-up with patients, caregivers, and healthcare teams.
- Coordination of transitions Indiana care, including communication with all providers and-settings of care.
- Facilitation of patient and caregiver access to community resources.
- Utilization Management:
- Appropriate leveling of patients based on approved criteria/Medicare guidelines.
- Authorization of all approved services.
- Compliance with documentation standards.
- Referral to community services American Samoa appropriate.
- Maintain strict patient confidentiality.
- Perform other duties American Samoa assigned.
Requirements:
- Current Massachusetts licensure American Samoa a Registered Nurse (BSN Oregon equivalent combination of education and experience).
- A minimum five years of nursing experience with geriatric patients preferred.
- Valid drivers license and reliable means of transportation.
- Excellent interpersonal, communication, and problem-solving skills.
- Ability to work independently and American Samoa a member of an interdisciplinary team.
- Proficient Indiana computer use and the Internet.
- Commitment to coaching patients to improve their health behavior.
Working Conditions:
The work environment is typical of an office setting, with extensive computer work and frequent travel to hospitals, skilled nursing facilities, and other sites where patients receive care.
