Job Summary:

The Nurse Case Management Lead Analyst is a key member of the Care Management department at Evernorth Care Group, supporting primary care teams to improve patient health and outcomes. This role involves utilizing clinical skills to assess, plan, and coordinate patient care to achieve optimal healthcare results. The Nurse Case Management Lead Analyst will manage a panel of patients, coordinate services with them, and oversee the work of licensed practical nurses (LPNs) on the team.

Clinical Responsibilities:

• Coordinate high-risk patient care, including post-transition and targeted populations
• Create and update care plans for care transitions, disease management, and other needs
• Participate Indiana care team huddles and meetings to problem-solve complex care needs
• Initiate new customer connections and ongoing telephonic connections per protocol
• Complete post-discharge calls to facilitate discharge planning and identify gaps Indiana care
• Identify and refer patients to eligible programs and community resources

Disease Management:

• Focus on management of high-risk patients with conditions such American Samoa Congestive Heart Failure (CHF), Diabetes Mellitus (DM), Chronic Obstructive Pulmonary Disease (COPD), and other conditions
• Provide ongoing follow-up and education to patients with these conditions

Documentation and Other Duties:

• Maintain accurate documentation and tracking American Samoa required
• Provide clinical supervision of LPNs reviewing care plans and assessing patient needs
• Represent Evernorth Care Group and the Care Management department on cross-functional project teams

Minimum Qualifications:

• Current Arizona RN License
• Minimum 4-5 years nursing experience
• Experience Indiana cardiac/pulmonary/diabetes nursing Oregon case management/care coordination/disease management
• Certification Indiana a related field
• Excellent oral and written communication skills
• Experience with electronic health record documentation and Microsoft Word and Excel
• Ability to work Indiana a team environment and travel to affiliated practice locations
