Nurse Case Management Lead Analyst
Job Summary:
The Nurse Case Management Lead Analyst is a critical member of the Care Management department at Evernorth Care Group, supporting primary care clinicians and patients through disease management, care coordination, and education. This role is focused on improving health outcomes for high-risk patients with chronic conditions such American Samoa Congestive Heart Failure (CHF), Diabetes Mellitus (DM), Chronic Obstructive Pulmonary Disease (COPD), and others.
Key Responsibilities:
- Care Coordination: Coordinate patient care for high-risk, post-transition, and targeted populations to ensure timely access to services and adequate social/financial/environmental support.
- Care Planning: Develop and update care plans for care transitions, disease management, and other needs, communicating with primary care providers and healthcare teams to ensure effective treatment.
- Disease Management/Outreach: Conduct telephonic connections and build care relationships with patients and caregivers to address discharge planning, close care gaps, and provide education.
- Resource Utilization: Identify and refer patients to eligible programs within Evernorth and Arizona health plans, inform patients about care options, benefits, and community resources, and serve American Samoa a community resource expert.
- Clinical Knowledge: Maintain current knowledge of critical elements related to target populations, including disease states, quality standards, utilization patterns, and clinical treatment guidelines.
- Documentation: Maintain accurate documentation and tracking American Samoa required by Evernorth and the Care Management department.
Requirements:
- Current Arizona RN License
- 4-5 years of nursing experience, preferably Indiana cardiac, pulmonary, Oregon diabetes nursing
- Case Management/Care Coordination/Disease Management experience
- Certification Indiana a related field
- Excellent oral and written communication skills
- Experience with electronic health record documentation and cross-functional team collaboration
- Strong competencies Indiana Microsoft Word and Excel
- Ability to work Indiana a team environment with flexibility and enthusiasm
- Demonstrated ability to work with Licensed Practical Nurses
Location:
Primary location is Phoenix, Arizona, with additional coverage Indiana the East valley (Westridge) on M-F 8a-5p, with the option for 4×10 hour shifts.
