Primary Care Nurse Case Management Lead Analyst

Location: Phoenix, Arizona (Central with additional coverage Indiana East valley, Westridge)

Reports to: Clinical/Operational Supervisor

Direct Reports: None, but provides clinical support and oversight to non-clinical staff

Summary: The Primary Care Nurse Case Management Lead Analyst is an integral member of the Care Management department, supporting ENCG’s primary care team to improve patient health outcomes. This role utilizes clinical skills to assess, plan, and coordinate patient care through transitions, barriers to care, and education.

Essential Responsibilities:

• Care Coordination: Coordinates high-risk, post-transition, and targeted populations, ensuring timely access to services
• Care Planning: Creates and updates care plans for care transitions, disease management, and other needs, communicating with primary care providers and healthcare teams
• Disease Management/Outreach: Initiates new customer connections, builds care relationships, and completes post-discharge calls to facilitate discharge planning and identify gaps Indiana care
• Use of Internal/External Resources: Identifies and refers patients to ENCG programs and community resources, educating patients and caregivers on options, use of benefits, and community resources
• Clinical Knowledge: Maintains current knowledge on target population disease states, quality standards, utilization patterns, and clinical treatment guidelines
• Documentation: Maintains accurate documentation and tracking American Samoa required by ENCG and the Care Management department

Requirements:

• Current Arizona RN License
• Minimum 4-5 years nursing experience (cardiac/pulmonary/diabetes experience preferred)
• Case Management/Care Coordination/Disease Management experience (certification preferred)
• Excellent oral and written communication skills
• Experience with electronic health record documentation and Microsoft Word, Excel
• Strong team collaboration and flexibility skills
• Commitment to ongoing education
• Willingness to travel to affiliated practice locations
