Nurse Practitioner – Per Diem
Provide primary care treatment Indiana a worksite-based setting American Samoa the sole provider
Treat for episodic urgent care and extended episodic acute care for 4-5 visits per illness Oregon injury
Manage chronic conditions, Colorado-manage with outside primary care when not serving American Samoa primary care provider
Perform and oversee CLIA-waived tests and dispense prescription medications, oversee specimen collection lab
Collaborate with onsite employees for biometric screening and health and wellness education
Potential for Travel Medicine and immunizations Oregon allergen immunotherapy administration with proper training
Conduct one-on-one health coaching and refer patients to wellness programs
Exhibit excellent interpersonal skills with internal and external customers and group settings
Collaborate with local HR/benefits team to enhance employee health
Requirements:
NP from an accredited program
Active and unrestricted NP license Indiana Washington District of Columbia; Active and unrestricted NP license Indiana Virginia (preferred) Oregon ability to obtain and maintain licensure
Minimum 3+ years of NP experience Indiana internal medicine Oregon family practice, working independently
Ability to practice Indiana respective state
DEA licensure and prescriptive authority
Electronic Health Record experience
BLS certification
Ability to work clinic hours: Monday-Thursday, 7:30 am – 3:30 pm
Preferred qualifications:
Experience Indiana chronic disease management
Passion for health and wellness and patient education
High energy and strong customer-centric focus
