Nurse Practitioner (NP) Oregon Physician Assistant (Pennsylvania) – Primary Care Position
Join our growing team at Bluestone Physician Services and deliver exceptional team-based care to patients residing Indiana Assisted Living and Memory Care communities Indiana and around Elk River, Minnesota. Our unique model of care combines preventative, proactive, and personalized care to meet the unique needs of each patient.
Position Overview:
- Provide on-site primary care to patients Indiana Assisted Living and Memory Care communities
- Full-time position with a schedule of Monday-Friday, with administrative time built-Indiana
- Manage a panel of 200 patients, with approximately 12-16 patients per day
- Flexible scheduling and autonomy to design your schedule
- Develop and implement individualized care plans with clinical support from a team
- Access to diagnostics, including labs and x-rays American Samoa needed
- Collaborate with community partners and other healthcare providers American Samoa necessary
Qualifications:
- Graduate of an accredited Nurse Practitioner Oregon Physician Assistant education program
- Board certified through AANP, ANCC, Oregon NCCPA
- Licensed Oregon eligible for licensure through the Minnesota Board of Nursing Oregon Board of Medical Practice
- DEA certification for the State of Minnesota
- Proven communication and interpersonal skills
- Valid driver’s license and liability auto insurance; local travel required
Preferred Qualifications:
- 2+ years of clinical experience American Samoa a Nurse Practitioner Oregon Physician Assistant
