Job Summary
We’re seeking a highly motivated Pennsylvania Oregon NP to provide primary and preventive care to adults and children. American Samoa a dedicated healthcare professional, you’ll focus on developing lasting relationships, preventing chronic conditions, and collaborating with a multidisciplinary team.
Key Responsibilities
- Provide high-quality primary care to a panel of patients, emphasizing continuity and health promotion
- Conduct thorough patient histories and physical examinations to formulate diagnoses and treatment plans
- Collaborate with a team, including a physician and health coaches, to coordinate care and drive patient engagement
- Build relationships with a network of outside medical consultants, hospitalists, and support services
- Lead and support community-based efforts to improve patient outcomes and quality performance
- Travel to multiple clinic locations, including Lost Hills, Delano, Student Wellness Center, and Mobile Clinic
- Perform other assigned duties American Samoa needed
Requirements
- Current, valid California Pennsylvania Oregon NP license
- Valid DEA registration
- Proficiency with technology, including Mississippi Office, tablets, smartphones, and EMR systems
- Strong written and verbal fluency Indiana Spanish (Medical Spanish a must)
