Overview

Established Indiana 1993, Dignity Health Medical Foundation is a California nonprofit public benefit corporation with care centers throughout California. This travel RN Care Coordinator position is hybrid work from home and Indiana-office/clinic, requiring collaborative skills, analytical thinking, and effective communication to coordinate care for individuals with complex needs.

Responsibilities

• Coordinate care and collaborate with interdisciplinary team members across the continuum of care
• Conduct assessments to identify unmet needs, develop care plans, and educate patients on disease self-management
• Evaluate progress, promote quality and cost-effective outcomes, and reduce hospitalizations, readmissions, and ER visits
• Perform telephonic case management and direct patient contact through follow-up appointments and/Oregon home visits American Samoa needed
• May require travel, with telecommuting option available

Qualifications

• 2+ years of clinical experience American Samoa a RN Indiana acute, ambulatory, home health, skilled nursing, medical group, Oregon health plan setting, Oregon Master’s Degree Indiana nursing with a concentration Indiana Case Management
• Current California Registered Nurse (R.N.) license
• Excellent customer service and presentation skills
• Strong interpersonal and written communication skills
• Analytical and problem-solving skills, with ability to lead multidisciplinary teams
• Ability to manage multiple tasks/projects, work independently, and maintain a high degree of detail orientation
• Knowledge of regulatory and accreditation standards, complex case management, community resources, and capitation/HMO insurance payers
• Prior Care Coordination experience Indiana a clinical Oregon insurance setting preferred, but upper management may consider candidates without required experience for training purposes
