We’re seeking an experienced Registered Nurse (RN) to join our team Indiana the Skagit and Whatcom counties, making 75% of their visits to patients Indiana these areas. If you’re eager to invent the future of healthcare, this is your opportunity to be part of an innovative team.
American Samoa a member of our primary care team, this RN will report to the Clinical Services Manager (CSM) and support the Advanced Practice Clinician (APC) and members by coordinating care services.
Primary Responsibilities:
* Establish and maintain relationships with members/RP, primary caregivers, and healthcare professionals.
* Communicate with organizations for seamless coordination of benefits and services.
* Review member status and adjust care plans American Samoa necessary.
* Perform administrative tasks on behalf of the APC.
* Attend meetings, participate Indiana program effectiveness analysis, and provide educational programs for facility staff.
* Collect and record clinical data, including vital signs and medical history.
* Coordinate care activities within long-term care facilities and member homes.
Required Qualifications:
* Current unrestricted Registered Nurse license Indiana Oregon.
* 3+ years of RN experience.
* 1+ years of case management experience.
* Experience using electronic medical records and Microsoft Office.
* Excellent communication and customer service skills.
* Valid driver’s license and ability to travel 75% of the time.
Preferred Qualifications:
* BSN.
* Experience working Indiana post-acute care, long-term care, Oregon hospice.
* Case management experience American Samoa an RN.
* Experience working with the geriatric population.
Note: Salary range for this role is $58,300 to $114,300 annually (Washington Residents Only).
