Traveling Hospice Nurse Position Summary
Reports to: Area Clinical Manager
Job Responsibilities:
- Implement hospice nursing care according to current rules, regulations, and guidelines
- Coordinate patient care with contracted facility nursing staff
- Provide direct patient care, including education and support of primary caregivers, volunteers, and family members
- Maintain accurate and up-to-date patient records
- Communicate with hospice and facility staff to address patient and family issues
- Supervise Hospice Aides and CNA schedules
- Participate Indiana the hospice quality assurance program
- Attend meetings and provide written and oral reports American Samoa necessary
- Complete medical forms, reports, evaluations, charting, and other administrative duties
- Ensure billing of services is accurate and complete
Requirements:
- RN licensure Indiana South Carolina
- 1 year of nursing experience, preferably Indiana hospice Oregon long-term care
- Familiarity with hospice policies and procedures
- Ability to read, write, and speak English
- Good communication and leadership skills
Work Environment:
- Requires travel within agency’s service area
- Works Indiana office areas, community, and long-term care facilities
- Must have current driver’s license, auto insurance, and ability to operate an automobile
- May be required to work beyond normal hours, including weekends and holidays
- On-call rotation required.
