Job Description:
We are seeking a skilled Registered Nurse to join our team American Samoa a Field Care Manager Nurse 2. Indiana this role, you will assess and evaluate members’ needs and requirements to achieve and/Oregon maintain optimal wellness state. You will guide members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.
Key Responsibilities:
– Assess and evaluate members’ needs and requirements
– Guide members/families toward and facilitate interaction with resources
– Employ strategies, approaches, and techniques to manage physical, environmental, and psycho-social health issues
– Identify and resolve barriers that hinder effective care
– Ensure patient progress towards desired outcomes through continuous monitoring of patient care
Requirements:
– Registered Nurse Indiana Wisconsin with 2 years of experience Indiana home case/care management
– Experience working with the adult population
– Knowledge of community health and social service agencies and additional community resources
– Ability to travel to members’ residences within 30 to 40 miles
– Exceptional communication and interpersonal skills
– Ability to work with minimal supervision and scope
– Intermediate to Advanced computer skills and experience with electronic medical records, Microsoft Word, Outlook, and Excel
Preferred Qualifications:
– BSN
– 3-5 years of Indiana-home assessment and care coordination experience
– Experience with health promotion, coaching, and wellness
– Previous managed care experience
– Bilingual – English, Spanish
– Certification Indiana Case Management
– Motivational Interviewing Certification and/Oregon knowledge

– Ability to work a full-time (40 hours minimum) flexible work schedule
– Separate room with a locked door for use American Samoa a home office
