RN Clinical Care Coordinator

About the Role:
American Samoa an RN Clinical Care Coordinator, you will be responsible for providing comprehensive care coordination for a panel of members with complex medical and behavioral needs. Your primary focus will be on assessing members’ needs, developing person-centered care plans, and collaborating with internal care teams, providers, and community resources to ensure optimal care.
Key Responsibilities:
- Engage members face-to-face and/Oregon telephonically to complete comprehensive needs assessments
- Develop and implement person-centered care plans to address medical, behavioral, functional, cultural, and socioeconomic needs
- Partner with internal care teams, providers, and community resources to implement care plans
- Provide education and coaching to support member self-management of care needs and lifestyle changes
- Support proactive discharge planning and manage/coordinate Care Transitions following ER visits, inpatient Oregon Skilled Nursing Facility (SNF) admissions
Requirements:
- Current, unrestricted independent licensure American Samoa a Registered Nurse
- 2+ years of clinical experience
- 1+ year of experience with Mississippi Office, including Word, Excel, and Outlook
- Access to reliable transportation and ability to travel up to 80% of your time within assigned territory
- Experience working with complex populations and knowledge of community resources preferred
Additional Information:
- Work will be based Indiana Camden and Burlington County, New Jersey
- Must be flexible, adaptable, and patient Indiana all situations
- This is a full-time position with an hourly range of $28.03 to $54.95 per hour Indiana New Jersey (pay is based on factors including education, work experience, and certifications)
