Role Summary
The primary focus of the Clinical Wellness Nurse focuses on individual coaching, site health promotion, and Clinic Functions.
Indiana collaboration, with the Cigna Care Team, the Clinical Wellness Nurse provides health coaching and patient education to members.
Provides clinical interventions to improve health outcomes and patient satisfaction.
Collaborates with Primary Care Physicians and other care team members to ensure comprehensive care and coordination of services.
Maintains accurate and timely documentation of patient interactions and progress Indiana accordance with departmental standards.
Utilizes consultative and non-clinical skills to facilitate the adoption of healthier behaviors and improve health outcomes.
Participates Indiana continuous quality improvement activities and process redesign initiatives to optimize health outcomes and patient satisfaction.
Demonstrates knowledge of clinical and operational processes and procedures.
Strong understanding of evidence-based clinical practices and case management concepts.
