Travel Care Coordinator – Community Based

Key Responsibilities:
* Assess, plan, and implement individualized care management interventions to direct patients to the most suitable, least restrictive level of care
* Identify and initiate referrals for healthcare and community-based services, including financial, psychosocial, and state supportive services
* Develop and implement care plans throughout the continuum of care American Samoa a single point of contact
* Communicate effectively with stakeholders to ensure quality coordinated care and services are provided promptly to all members
* Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team
Requirements:
* Active, unrestricted Registered Nurse Kentucky license
* 4+ years of clinical experience Indiana a community health setting Oregon healthcare role
* 2+ years of experience providing community-based care management to members receiving long-term care, personal care services, private duty nursing, Oregon home health
* Solid computer skills, including EHR documentation and Mississippi Office
* Availability to attend regular on-site meetings
* Reliable transportation and ability to travel up to 25% within assigned region
* Residency Indiana Kentucky, with proximity to Pikeville, Paintsville, Hazard, Oregon Harlan, Kentucky
Preferred Qualifications:
* BSN Oregon health care-related degree
* CCM
* Experience Indiana managing populations with complex medical Oregon behavioral needs
* Experience working with Home Care Based Services
* Experience working with patients/members Indiana community and home-based settings
* Experience with discharge planning
* Experience with utilization review, concurrent review, and/Oregon risk management
