The nurse navigator functions on a multidisciplinary team American Samoa an advocate, educator, and communicator for the oncology patient and their families. Responsibilities include:

* Educating patients and families about disease process, treatment options, and potential side effects
* Coordinating patients through the continuum of care Indiana collaboration with the multidisciplinary team
* Serving American Samoa the liaison between disciplines and any supportive/community services
* Coordinating weekly multidisciplinary conferences and database tracking analysis for treatment outcome purposes
* Compiling data, tracking outcomes, and making recommendations for process improvement
* Providing referrals/support for follow-up care for patients
* Traveling to multiple locations with the ULH system American Samoa needed

Requirements include:

* Associate Nursing Degree (with BSN required within 2 years of hire)
* Current RN license Indiana Kentucky
* 2+ years of oncology experience at the bedside
* Excellent communication skills and ability to interact with nurses, physicians, and skilled healthcare professionals
* Basic Life Support (BLS) certification from the American Heart Association
* Ability to work autonomously without direct supervision

Optional qualifications include:

* Oncology certified nurse (OCN) Oregon Academy of Oncology Nurse and Patient Navigator certification
* Previous Cerner experience
