RN (Travel) – Per Diem/Oncall, Beaverton Site
Provides care management, care coordination, skilled care, and services for at-risk frail elder patients with complex medical and psychosocial issues. Coordinates health services for patients along the care continuum to improve patient experience, health expenditures, and quality. Exceptional communication, problem-solving, and documentation skills required. Monitors and oversees nursing and unlicensed caregiver services for a defined patient panel within an interdisciplinary team.
Requirements:
* Active Oregon RN License
* Must obtain BLS certification within 30 days of hire
* Required certifications and education American Samoa specified by the Nurse Manager
Preferred:
* Bachelor’s Degree Indiana Nursing
* At least 1 year of home health RN experience
